Total Care Therapy in Central Ohio is seeking a compassionate Physical Therapist to evaluate, develop, and implement individualized treatment plans that improve mobility and independence for residents in assisted living communities.

You will collaborate with an interdisciplinary team, supervise PTAs where allowed, document using EMR, and help residents reduce pain while maintaining quality of life. This full-time role offers a therapist-owned culture and opportunities for professional growth.

Qualifications

• Current State Physical Therapist License is required.
• CPR/BLS certification required.
• Valid driver's license and reliable transportation.
• Experience in geriatrics, home health, assisted living, or skilled nursing is preferred but not required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments including exercise, gait and balance training.
• Reassess progress and modify treatment plans as needed.
• Supervise PTAs in accordance with state regulations where allowed.
• Document accurately using the EMR system.
• Educate residents, caregivers and staff on treatment plans and home programs.
• Collaborate with interdisciplinary team for coordinated patient care.
• Maintain a productive clinical caseload and deliver quality therapy.

Compensation: Salary (based on years of experience) Scheduling: Monday-Friday Full-Time (Weekends available but not required)

About the Role We are seeking a compassionate and motivated Physical Therapist to join our growing team. In this role, you will evaluate, develop, and implement individualized treatment plans that help residents improve mobility, restore function, reduce pain, and maintain independence.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.

Qualifications

• Current State Physical Therapist (PT) License
• Current CPR/BLS Certification
• Valid driver's license, reliable transportation, and auto insurance
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
