Job summary

Concordia Visiting Nurses is seeking a dedicated Physical Therapy Assistant (PTA) to join our home health team. You will work with Physical Therapists to help seniors regain strength and mobility in the comfort of their homes.

The role offers autonomy, one-on-one patient care, and opportunities for professional development within a faith-based health system. Prior PA PTA licensure and CPR may be required.

Qualifications

• Licensed Physical Therapy Assistant (PTA) with current credential.
• CPR certification or ability to obtain prior to employment.
• Compassionate communicator with strong teamwork.

Responsibilities

• Provide PTA services under the supervision of a licensed Physical Therapist with the established care plan.
• Assist patients to improve strength, balance, mobility, and independence at home.
• Implement therapeutic exercises, gait training, and mobility training.
• Educate patients and caregivers on exercise programs, fall prevention, and safe mobility.
• Observe, document, and report patient progress to supervising PT.
• Collaborate with nurses and other healthcare professionals to ensure high-quality patient care.

What you'll do as a Physical Therapy Assistant (PTA):

• Provide physical therapy treatments under the direction of a licensed Physical Therapist in accordance with the established plan of care.
• Assist patients in improving strength, balance, mobility, endurance, and functional independence.
• Implement therapeutic exercises, gait training, transfer training, and other evidence-based interventions to support recovery.
• Educate patients and caregivers on exercise programs, fall prevention, safe mobility techniques, and the proper use of assistive devices.
• Observe, document, and report patient progress while communicating any changes to the supervising Physical Therapist.
• Collaborate with nurses, occupational therapists, speech therapists, physicians, and other healthcare professionals to provide exceptional patient care.

Who you are as a Physical Therapy Assistant (PTA):

• Detail-oriented and organized with strong clinical observation and documentation skills.
• Comfortable working independently while remaining an active member of an interdisciplinary healthcare team.
• Graduate of an accredited Physical Therapist Assistant program with a current Pennsylvania Physical Therapist Assistant (PTA) license, or the ability to obtain one prior to starting.
• Current CPR certification or the ability to obtain prior to employment.
