Job summary

Evolve Therapy Services, LLC is seeking a dedicated Physical Therapy Assistant to join our in-house skilled nursing facility team in Rochester, MN. The role supports the Physical Therapist, conducts screenings, and helps implement individualized treatment plans to improve residents’ mobility and quality of life.

This full-time position offers a $5,000 sign-on bonus plus tuition reimbursement, with additional benefits and options for part-time or PRN schedules.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in evaluating resident progress under the supervision of the Physical Therapist.
• Develop and modify treatment plans with the Physical Therapist and other team members.
• Conduct individual and group therapies focusing on strength, mobility, balance, and ROM as outlined in the plan.

Education
