Job summary

Interim HealthCare in South Bend, IN is seeking a Home Health Physical Therapist Assistant (PTA) to provide compassionate, home-based therapy and support to patients in their own homes. You will work as part of a home health team focused on the patient’s plan of care, goals, and mobility improvements.

The ideal candidate is a graduate of an accredited PTA program with an active Indiana PTA license and CPR certification, with at least 1 year of PTA experience, preferably in home healthcare.

Qualifications

• Must be a graduate of an accredited PTA program and hold an active Indiana PTA license.
• CPR certification is required.
• Strong clinical judgement and interpersonal skills are essential.

Responsibilities

• Provide home-based physical therapy to patients with mobility and functional challenges.
• Collaborate with a home health team (RN, LPN, CNA, HHA, OT, SLP) to implement the plan of care.
• Observe movements, assess progress, and modify the care plan as needed.
• Assist patients with exercises and hands-on therapy to improve mobility and reduce pain.
• Educate patients on their plan of care, goals, exercises, and self-care.
• Coach family members on the plan of care and safely assisting with exercises.

As a Home Health Physical Therapist Assistant, here’s a big-picture view of what you’ll do:

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals
• Observe movements, conduct assessments, document progress and modify care plan as needed
• Assist patient with exercises and hands-on therapy to manage pain and increase mobility
• Educate patients on their plan of care, established goals, exercises and self-care
• Coach family on the plan of care, therapy goals and safely assisting with exercises

A few must-haves for Home Health Physical Therapist Assistants:

• Graduate of an accredited Physical Therapist Program and active PTA license in Indiana
• Minimum of 1 years of physical therapy assistant experience, ideally in home healthcare
• CPR certification
• Knowledge of state and federal home health regulations
• Good clinical judgement, strong interpersonal skills, dexterity and compassion
