Job summary

Interim HealthCare in Miami-Dade is seeking a Home Health Physical Therapist Assistant (PTA) to deliver patient-centered care in the home setting. You’ll assess mobility, implement plan of care, and collaborate with a home health team to achieve personalized goals.

The role emphasizes flexible assignments, one-on-one patient contact, and ongoing education through online training and CEUs. Active FL PTA license, CPR, and prior home health experience are required.

Qualifications

• Active Florida PTA license.
• CPR certification.
• Minimum 1 year of physical therapy assistant experience, ideally in home healthcare.
• Knowledge of state and federal home health regulations.
• Good clinical judgment and strong interpersonal skills.

Responsibilities

• Provide home-based physical therapy to patients to restore mobility and function.
• Collaborate with RN, LPN, CNA, HHA, OT and SLP on plan of care.
• Assist patient with exercises and hands-on therapy.
• Educate patients on plan of care, goals, exercises and self-care.
• Coach family on plan of care and safety during exercises.

As a Home Health Physical Therapist Assistant, here’s a big-picture view of what you’ll do:

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals
• Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s)
• Assist patient with exercises and hands-on therapy to manage pain and increase mobility
• Educate patients on their plan of care, established goals, exercises and self-care
• Coach family on the plan of care, therapy goals and safely assisting with exercises

• Active PTA license in FL
