Summary
The Physical Therapist Assistant assists in the implementation of treatment programs and plan of care as directed by the licensed Physical Therapist, in accordance with State Practice Act.
- Provides physical therapy treatments to patients/residents as determined through the therapy plan of care, established by the Physical Therapists, and State Practice Act.
- Gathers data for screens and reports the results to the supervising Physical Therapist.
- Records data on patient/resident responses to treatments and communicates to the Physical Therapist overseeing the treatment plan.
- Documents up-to-date services provided to patients/residents, includes daily progress note, weekly summary, and therapy summary sheet.
- Participates in conferences, consultations, and educational programs designed to improve patient/resident care and staff performance as directed by the Director of Rehabilitation.
- Instructs patient/resident, family, and/or caregivers in physical therapy recommendations to be continued upon patient/resident discharge as directed by the Physical Therapist.
- Assists in the assurance that the physical surroundings and equipment are sanitary, orderly and free from conditions that may cause safety hazards.
- Participates in scheduled in-service programs related to work topics as directed by the Director of Rehabilitation.
- Working with the Physical Therapist and Director of Rehabilitation assists in the supervision and education of affiliating students as allowed by State Practice Act.
- Performs other duties as assigned and allowed by State Practice Acts and regulation.
- Demonstrates working knowledge of and compliance with the facility Compliance program, policies and procedures, applicable federal and state laws, rules and regulations.
- Demonstrates compliance with the protection of patient/resident health information and company sensitive information including compliance with company policies and procedures, applicable federal and state laws, rules, and regulations.

Qualifications
- Graduate of an APTA accredited physical therapist assistant program and eligible for licensure in the state in which the program is located.
- Prior experience in long-term care is preferred.
- Ability to work flexible work hours to support state and federal resident care requirements.
Language Skills
Able to communicate through verbal and written mode, in English.
