Job Position: Physical Therapy Assistant for an In-house Skilled Nursing Facility.
*Must have active Wisconsin Physical Therapy Assistant license.

Job Type: PRN with CONSISTENT HOURS!

Job Purpose: To assist in planning, organizing, developing, and directing Physical Therapy services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as directed by the Administrator, to ensure the highest quality resident care.

Job Functions:

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine the need for intervention/treatment.
• Assist in evaluations under the direction of the Physical Therapist.
• Help identify goals and develop treatment plans.
• Conduct individual and group treatments, including muscle strength, mobility, balance, gait training, and modalities.
• Assist in selecting and adapting physical therapy techniques/media.
• Monitor residents’ responses and consult with the Physical Therapist for modifications.
• Establish and modify resident schedules in collaboration with the team.
• Train residents, families, and staff in equipment use.
• Participate in education for residents, families, and staff.
• Maintain sound judgment in therapy programs.
• Report to work on time, ensuring productivity and efficiency.
• Participate in Quality Assurance programs.
• Complete documentation in NetHealth EMR systems as per CMS requirements.

Education & Requirements:

• Associates Degree in Physical Therapy from an accredited program.
• Maintain ongoing continuing education and professional development.
• Must have an active Wisconsin Physical Therapy Assistant license.
