Job Title: Physical Therapist Assistant (PTA) – Long-Term Care

Department: Therapy

Reports To: Director of Rehabilitation

FLSA Status: Non-Exempt

Position Summary:

The Physical Therapist Assistant (PTA) provides rehabilitative services under the direction and supervision of a licensed Physical Therapist in a long-term care facility. The PTA helps residents improve mobility, strength, coordination, and quality of life through individualized therapy plans while ensuring compliance with all regulatory and professional standards.

Essential Responsibilities:

Carry out treatment plans established by the Physical Therapist in accordance with resident goals and physician orders.

Assist residents in performing therapeutic exercises and activities designed to improve functional independence.

Observe residents during treatment and report any unusual reactions or changes in condition to the Physical Therapist and nursing staff.

Document treatment provided, resident progress, and responses to therapy in accordance with state and federal regulations.

Collaborate with the interdisciplinary care team to support the overall rehabilitation and wellness of each resident.

Educate residents, family members, and caregivers on techniques to support continued progress and safety.

Ensure the therapy gym, equipment, and tools are clean, maintained, and in good working order.

Follow facility policies, procedures, and infection control guidelines.

Requirements
Qualifications:

Graduate of an accredited Physical Therapist Assistant program.

Current and valid state licensure or certification as a Physical Therapist Assistant.

Prior experience in long-term care, geriatrics, or skilled nursing preferred.

CPR certification (required or must be obtained upon hire).

Excellent interpersonal and communication skills.

Ability to work independently and as part of a team.

Physical Requirements:

Ability to stand, walk, bend, squat, and reach throughout the workday.

Must be able to safely assist with lifting and transferring residents.

Capable of lifting up to 50 pounds as needed.

Work Environment:

This position operates in a long-term care/skilled nursing facility.

May be exposed to infectious diseases, odors, and other resident-related conditions.
