Job summary

Madison Park Healthcare is seeking a Physical Therapist Assistant (PTA) for a long-term care setting. The PTA provides rehabilitative services under the supervision of a licensed Physical Therapist to enhance mobility, strength, coordination and quality of life for residents.

Responsibilities include executing treatment plans, guiding residents through therapeutic exercises, documenting progress, and collaborating with the care team to support overall wellness.

Qualifications

• Graduate of an accredited PTA program.
• Current and valid state licensure or certification as a PTA.
• CPR certification required or to be obtained upon hire.
• Prior experience in long-term care, geriatrics, or skilled nursing preferred.

• Carry out treatment plans established by the Physical Therapist in accordance with resident goals and physician orders.
• Assist residents in performing therapeutic exercises and activities designed to improve functional independence.
• Observe residents during treatment and report any unusual reactions or changes in condition to the Physical Therapist and nursing staff.
• Document treatment provided, resident progress, and responses to therapy in accordance with state and federal regulations.
• Collaborate with the interdisciplinary care team to support the overall rehabilitation and wellness of each resident.
• Educate residents, family members, and caregivers on techniques to support continued progress and safety.
• Ensure the therapy gym, equipment, and tools are clean, maintained, and in good working order.
• Follow facility policies, procedures, and infection control guidelines.

Position Summary

The Physical Therapist Assistant (PTA) provides rehabilitative services under the direction and supervision of a licensed Physical Therapist in a long-term care facility. The PTA helps residents improve mobility, strength, coordination, and quality of life through individualized therapy plans while ensuring compliance with all regulatory and professional standards.

Requirements
