Job summary

Creekside Care Center of Gahanna, part of Paradigm Healthcare, is hiring a Full-Time Physical Therapist Assistant (PTA) to join our rehab team in Gahanna, OH. You will work under the supervision of a licensed PT to improve mobility, strength and function for residents in a skilled nursing setting.

We use Net Health documentation and collaborate with an interdisciplinary team to deliver patient-centered care in a supportive, team-focused environment.

Qualifications

• Graduate of an accredited Physical Therapist Assistant (PTA) program.
• Current PTA license in the state of Ohio (or eligible for licensure).
• Experience in Skilled Nursing Facility (SNF) or Long-Term Care (LTC) preferred, but not required.

Responsibilities

• Provide physical therapy treatments under the direction of a licensed Physical Therapist.
• Assist in implementing individualized treatment plans to improve mobility, strength, and function.
• Document patient progress and treatments accurately using Net Health.
• Communicate effectively with therapists, nursing staff, and interdisciplinary team members.
• Educate patients and caregivers on therapeutic exercises and mobility techniques.
• Maintain compliance with facility policies and regulatory standards.
