Job summary

Boost Home Healthcare is seeking a licensed Physical Therapist to provide physician prescribed PT in patients' homes in Fort Collins, CO. You will assess, develop and modify plans of care, coordinate with the care team, and supervise field staff as needed.

The role requires state licensure, and a preference for 1 year in home health with Medicare OASIS documentation experience. Flexible scheduling and patient-centered care are emphasized.

Qualifications

• Graduate with a Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program.
• Currently licensed in the state in which practicing.
• One year of home health experience preferred.
• Knowledgeable and experienced with Medicare OASIS documentation.

Responsibilities

• Provide physician prescribed physical therapy.
• Improve or minimize residual physical disabilities of the patient.
• Return the individual to optimum and productive level within the patient’s capability.
• Participate with all other home care personnel in patient care planning.
• Direct and supervise personnel as required.
• Responsible for initial assessment, plan of care, maintenance program development and modifications and reassessments every 30 days.

• Graduate with a Bachelor's, Master's, or Doctorate in physical therapy from a physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy Association.


• Currently licensed in the state in which practicing


• One (1) year experience preferred in a home health setting

Job Type:

• PRN, Part-time and Full-time

Compensation and Schedule:

• 68-76$ per visit


• Monday through Friday


• Day Shift


• Weekends as needed
