Job summary

Healthcare Nursing of Denver is seeking a Physical Therapist Assistant to join an in-house therapy team in Colorado. The PTA will provide rehabilitative therapy for patients referred with cognitive and/or ADL concerns, ensuring functional improvement and adherence to clinical standards.

The role reports to the Director of Rehabilitation Services and involves collaboration with physicians, nurses, and therapists.

Qualifications

  • Colorado PTA license is required.
  • Ability to supervise PTAs and rehabilitation aides.
  • Experience in cognitive/ADL rehab is preferred.

Responsibilities

  • Provide rehabilitative therapy to patients per referrals.
  • Follow regulatory and clinical practice standards.
  • Document objective measures and justify continued services.
  • Screen/evaluate patients and develop care plans.
  • Use standardized assessments to support interventions.
  • Deliver rehabilitative, skilled modalities per plan of care.

Principal Responsibilities

The Physical Therapist Assistant is responsible for providing rehabilitative therapy to patients upon referral for cognitive and/or ADL physical functioning disorders following all regulatory and clinical practice requirements. This includes recovery, improvement in function and when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services.

This position reports to the Director of Rehabilitation Services.

  • Effectively screens/evaluates patients and/ with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
  • Utilizes standardized assessments and evidence-based practice to support clinical interventions.
  • Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
  • Administers various procedures as part of the rehabilitation plan including but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
  • Assesses patient's progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
  • Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
  • Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
  • Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
  • Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
  • Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy.
  • Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
  • Participation in required meetings including but not limited to: patient care conferences, utilization review meetings, rehabilitation meetings, etc.
  • Participate in facility and company required trainings, in-services and conferences.
  • Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.
  • Adherence to Company required policy and procedures and code of conduct.
  • Active State Licensure
