Qualifications

• Home health experience preferred.

Responsibilities

• Maintain records and clinical notes within 24 hours of patient care.

• PHYSICAL THERAPIST ASSISTANT - Home Health & Hospice: Experience the opportunity to provide compassionate, personalized care in the home. Our skilled staff improve lives by providing care for our patients wherever they call home. We focus on caring for patients recovering from injuries, living with a chronic illness, and other medical needs.

Responsibilities

• Provide physical therapy services under the direction of the physical therapist and according to the physician's plan of care.
• Treat patients to relieve pain, develop or restore function, and maintain maximum performance.
• Assist patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities, and prosthetic training.
• Utilize physical agents such as heat, cold, water, light, electricity, ultrasound, and massage.
• Observe and report patient reactions and condition changes to the physical therapist.
• Instruct patients in the use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.
• Maintain records and clinical notes within 24 hours of patient care.
• Participate in team meetings and in-service education programs.
• Perform other duties as assigned.

Qualifications

• Graduate of an accredited physical therapist assistant program with an Associate’s degree approved by the American Physical Therapy Association.
• Current BLS-CPR certification.
• Home health experience is preferred.

Pay Range

$21.91 - $31.77/hour
