Qualifications

Responsibilities

• Implement individualized treatment plans developed by the Physical Therapist.
• Assist residents with therapeutic exercises, gait training, balance activities, mobility training, and strengthening programs.
• Monitor resident progress and communicate changes to the supervising Physical Therapist.
• Accurately document treatments, resident responses, and progress notes in compliance with facility and regulatory standards.
• Educate residents and caregivers on exercises, mobility safety, and therapy goals.
• Collaborate with nursing staff, physicians, and interdisciplinary care teams to ensure coordinated resident care.
• Maintain a safe, clean, and organized therapy environment.
• Promote resident dignity, comfort, and independence throughout therapy sessions.
