Job description

MGA Homecare has been providing Private Duty Nursing, Skilled Nursing Visits, Occupational Therapy, Physical Therapy, and Speech Therapy services within the home and community-based setting for over a decade. In 2022 MGA opened MGA Behavior Therapy, offering Applied Behavior Analysis (ABA) and early intervention for pediatric patients with autism spectrum disorder (ASD) and other developmental disabilities.

Job Summary

Physical therapists are licensed professionals who work with patients that have sustained limitations in their overall physical function. They examine, evaluate, diagnose, develop treatment plans and provide prognosis for each patient individually. The goal is to restore function, improve mobility, and reduce pain to re-establish a patient’s prior functional level.

Duties and Responsibilities
  • Seeks and obtains sufficient information necessary to function effectively where assigned.
  • Provides individual patient/client services and determines priority needs for physical therapy by:
    • Assessing and evaluating therapeutic, rehabilitative and functional status.
    • Assessing for muscle strength, mobility, gait, ROM, potential for rehabilitation and evaluation of the home environment.
    • Participating in the development of the total plan of care.
    • Directing the physical therapy treatment plan of care.
    • Regularly re-evaluating patient/client progress and revising the plan of care as necessary.
  • Initiates the physical therapy program and instructs other personnel in certain phases of physical therapy in which they may work with a patient/client.
  • Instructs personnel in the use and care of therapeutic appliances and/or prosthetic devices when applicable.
  • Maintains communication regarding patient/client status with supervisory personnel.
  • Collaborates with the physician and team members to develop, implement and support the patient’s plan of care.
  • Completes and maintains accurate clinical notes regarding patient evaluation and attainment of goals.
  • Observes the patient for reactions to treatment, changes in condition and reports changes as necessary.
  • Assists the caregivers/families, other healthcare personnel in providing continuity of care.
  • MGA Policies, Procedures and Handbook are adhered to.
    • Notes are completed accurately and submitted as per respective policies and procedures.
    • Clinical documentation must be turned in weekly per agency guidelines.
    • Represents themselves in a positive and professional manner in the company and community.
    • Adheres to dress code with a clean and neat professional appearance.
    • Reports on time and as scheduled in order to complete work within designated time.
    • Participation in Agency’s other mandated activities as requested by Agency Director such as In‐services.
Requirements
  • Bachelor’s, Master’s, or Doctorate degree from an accredited therapy program.
  • Professional experience required per state regulations.
  • Current License and in good standing with State’s Licensing Board.
  • Current CPR certification per state regulations.
  • Successful completion of appropriate screening tests with a passing rate.
  • Complies with accepted professional standards of practice.
  • Displays good observation skills and judgment as well as good oral and written communication skills.
  • Reliable transportation.
Physical Requirements
  • Ability to regularly sit, stand, stop, bend, kneel, walk, balance, crouch, crawl, push, pull and reach with hands and arms and other physical activities as deemed necessary by job duties.
  • Vision and hearing capabilities within normal limits, with or without aid.
  • Ability to frequently lift and/or move up to 15 pounds and occasionally lift or move 100 pounds or more if applicable.
  • Ability to perform the anticipated level of physical assist required to provide safe client care. If the anticipated level of assist greater than that which the employee can safely provide, the supervisor is to be notified immediately.
Work Environment
  • The work environment is highly variable, as care is generally provided in private homes.
  • Lighting, acoustics, air quality, temperature and environment hazards are not predictable.
  • The employee is occasionally exposed to wet and/or humid conditions, household pets/animals, fumes or airborne particles, and toxic or caustic chemicals.
  • The employee is required to travel frequently to complete home health visits.
