Job summary

CareCo Medical Home Health is seeking a licensed Physical Therapist to provide skilled in-home therapy services to patients in the community. This per diem/per visit role offers independent work with collaboration from an interdisciplinary team in Connecticut.

The ideal candidate will have a PT license in CT and a minimum of three years of healthcare experience, with home health and OASIS experience preferred. Mileage reimbursement and flexible scheduling are offered where applicable.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current CT PT license.
• Minimum of three years in healthcare.
• Home health and OASIS experience preferred.
• Strong communication and documentation skills.
• Ability to work independently with minimal supervision.
• Valid driver's license and insured vehicle.

Responsibilities

• Perform physical therapy evaluations, assessments, and skilled treatments in the home.
• Complete initial and comprehensive assessments, including OASIS, within required timeframes.
• Develop, implement, and update individualized Plans of Care.
• Document all visits, findings, and outcomes in a timely manner.
• Communicate changes in patient status to physicians, case managers, and the care team.
• Provide patient and caregiver education to promote recovery and safety.
• Participate in discharge planning and interdisciplinary care coordination.
• Supervise Physical Therapy Assistants in accordance with agency and regulatory guidelines.
• Promote patient safety and comply with all agency, state, and federal standards.

We manage acute and chronic conditions, provide wound care, medication management, and comprehensive therapy services.

Position Summary

The Physical Therapist plans and provides physical therapy services to patients in their homes in accordance with the Plan of Care and agency policies. This role may also include case management for patients requiring skilled therapy services only. Therapists work collaboratively with nurses and other disciplines to provide care, education, and support to patients and their caregivers under the supervision of the Clinical Supervisor.
