Job summary

CareCo Medical Home Health is seeking a licensed Physical Therapist to provide in-home therapy services to patients in their homes in Connecticut. The role includes independent work with a supportive interdisciplinary team and case management for skilled therapy needs.

The Per diem / per visit pay structure rewards evaluative and treatment performance with mileage reimbursement and flexible scheduling, while ensuring compliance with all state and federal standards and ongoing professional

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in Connecticut.
• Minimum of three years of healthcare experience.
• Home health and OASIS experience preferred.
• Strong communication and documentation skills.
• Ability to work independently with minimal supervision.
• Valid driver's license and reliable, insured vehicle.

Responsibilities

• Perform home-based physical therapy evaluations and treatments.
• Complete initial and comprehensive assessments including OASIS.
• Develop, implement and update individualized Plans of Care.
• Document visits, findings and outcomes promptly.
• Communicate changes in patient status to the care team.
• Educate patients and caregivers to promote recovery and safety.
• Participate in discharge planning and interdisciplinary care coordination.
• Supervise PT Assistants according to guidelines.
• Promote patient safety and comply with standards.

Position Summary

The Physical Therapist plans and provides physical therapy services to patients in their homes in accordance with the Plan of Care and agency policies. This role may also include case management for patients requiring skilled therapy services only. Therapists work collaboratively with nurses and other disciplines to provide care, education, and support to patients and their caregivers under the supervision of the Clinical Supervisor.

Key Responsibilities

• Perform physical therapy evaluations, assessments, and skilled treatments in the home
