Responsibilities

• Perform home visits for patients in a home care setting
• Observe, record, and coordinate care with multi-disciplinary team regarding the patient's treatment and any change in condition
• Teach, supervise, and counsel the patient and family regarding home therapy procedures and other care needs as appropriate to the patient’s condition

Qualifications

• Current license as a Physical Therapist in Connecticut
• Valid driver’s license
• Reliable transportation and willingness to travel in New London County
• Strong communication and interpersonal skills
• Home health experience including Medicare preferred
