Helping U Home Care is seeking compassionate Physical Therapists to provide high-quality home visits across the NYC metro area. You will work with patients in their homes, following the Plan of Care, with flexible scheduling and your choice of days.

Requirements include NYS PT license, at least 1 year of home health experience, and ability to travel to multiple NY counties. Benefits include paperless documentation, continuing education, referral bonuses and daily pay.

Formación

• Current and clear NYS Physical Therapy license.
• Minimum 1 year home health experience.
• Ability to travel to at least one NYS county (Manhattan, Brooklyn, Bronx, Queens).
• Monitor patients and report per Plan of Care.

Responsabilidades

• Provide PT visits to patients in their homes according to each patient's Plan of Care.
• Maintain documentation in paperless system as required.
• Communicate with patients and family/caregivers about progress and plan of care.
• Coordinate with care team to ensure seamless home-based therapy services.

Helping U Home Care is a locally owned, NYS Licensed Home Care Agency that specializes in providing High-Tech Home Care services.

Do you have home care experience, providing Home Care PT visits to patients in their homes?

We are looking for compassionate, dedicated and well-rounded Physical Therapists who can uphold our standards and culture and provide the highest quality available in the New York Metropolitan area

You can choose how many days each week you want to work and will be given all the PT visits for each assigned patient (according to each patient's Plan of Care).

Qualifications:

• > Current and clear NYS Physical Therapy license
• > Minimum 1 year home health experience
• > Ability to travel in a minimum of 1 New York State County (we have cases in Manhattan, Brooklyn, Bronx and Queens)
• > Monitor patients and report on their condition as per patient’s Plan of Care
