Job summary

Helping U Home Care in the New York metropolitan area seeks compassionate Physical Therapists to provide high-quality home health PT services across all five boroughs and surrounding counties. You can choose how many days per week you want to work and will receive all PT visits for assigned patients according to each Plan of Care.

Requires NYS PT license and at least 1 year of home health experience; travel to at least one NY county; benefits include flexible schedules, paperless documentation,

Qualifications

• Current and clear NYS Physical Therapy license.
• Minimum 1 year home health experience.
• Ability to travel to at least one New York State County.
• Monitor patients and report on their condition as per patient’s Plan of Care.

Responsibilities

• Provide physical therapy visits in patients' homes per Plan of Care.
• Monitor patient progress and report changes to care team.
• Travel to assigned counties within New York State to provide home health PT services.

Do you have home care experience, providing Home Care PT visits to patients in their homes?

We are looking for compassionate, dedicated and well-rounded Physical Therapists who can uphold our standards and culture and provide the highest quality available in the New York Metropolitan area

You can choose how many days each week you want to work and will be given all the PT visits for each assigned patient (according to each patient's Plan of Care).
