Job summary

Sweetwater Care in Fresno, CA is seeking a Licensed Physical Therapist to consult with patients, diagnose movement dysfunction, and develop individualized treatment plans. You will coach patients and teach therapeutic massage as part of their healing, and document goals and progress in the clinical record.

You will assess progress, adjust treatments, communicate with the health team, provide discharge summaries, and ensure compliance with regulatory requirements.

Qualifications

• Licensed Physical Therapist (state licensure required).
• Ability to supervise PT assistants, rehab aides, and students.

Responsibilities

• Consult with patients to learn about their condition and review medical history and referrals.
• Diagnose movement dysfunction by observing patients as they move.
• Develop individualized treatment plans.
• Coach patients and teach therapeutic massage to ease pain and promote healing.
• Maintain patient records and track progress toward goals.
• Use standardized assessments and evidence-based practice to support interventions.
• Assess progress and adjust treatments in the clinical record per requirements.
• Evaluate treatment outcomes and adjust services accordingly.
• Provide counseling to patients and families and coordinate with other health providers.
• Communicate with supervisor and health team about patient progress and plans.
• Provide comprehensive discharge summaries as required.
• Document all required information in electronic systems and maintain patient records.
• Participate in trainings and required in-services.
• Able to supervise and support staff and students.
• Adhere to company policies and code of conduct.

Principal Responsibilities

• Consult with patients to learn about their physical condition and symptoms. Review their medical history and referrals from doctors or surgeons.
• Diagnose movement dysfunction by listening to patients and observing them as they move.
• Develop individualized treatment plan
• Coach patient and teach them therapeutic massage to ease patients’ pain and provide stimulation to promote healing
• Maintain patient record by keeping track of goals
• Utilize standardized assessments and evidence-based practice to support clinical interventions.
• Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
• Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
• Participate in facility and company required trainings, in-services and conferences.
• Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.
• Adherence to Company required policy and procedures and code of conduct.
• Active State Licensure
