Job summary

Sweetwater Care is seeking a licensed Physical Therapist to assess movement dysfunction, develop individualized treatment plans, and guide patients through therapeutic massage to ease pain and promote healing.

You will maintain comprehensive patient records, use standardized assessments, and collaborate with a health team to optimize outcomes. Supervisory responsibilities include supporting PT assistants and students, with adherence to company policies and regulatory requirements.

Qualifications

• Active State Licensure
• Experience supervising physical therapy staff (PTAs, aides) preferred

Responsibilities

• Consult with patients to learn about their physical condition and symptoms; review medical history and referrals.
• Diagnose movement dysfunction by listening to patients and observing them as they move.
• Develop individualized treatment plans.
• Coach patients and teach therapeutic massage to ease pain and promote healing.
• Maintain patient records and track goals.
• Utilize standardized assessments and evidence-based practice to support interventions.
• Assess progress and adjust treatments in the clinical record per regulatory and clinical requirements.
• Evaluate treatment outcomes and modify services; refer as needed.
• Provide consultation to patients, families, caregivers and other service providers related to physical disorders.
• Communicate progress, barriers and plans with supervisor and health team members.
• Provide comprehensive discharge summaries per regulatory requirements.
• Document all required information in electronic systems and maintain patient records.
• Participate in required trainings, in-services, and conferences.
• Able to supervise PT assistants; rehabilitation aides and/or students.
• Adhere to company policies and code of conduct.
• Active State Licensure

• Maintain patient record by keeping track of goals
• Utilize standardized assessments and evidence-based practice to support clinical interventions.
• Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
• Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
• Participate in facility and company required trainings, in-services and conferences.
• Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.
• Adherence to Company required policy and procedures and code of conduct.
• Active State Licensure
