Job summary

Sweetwater Care is seeking a licensed Physical Therapist to evaluate patients, diagnose movement dysfunction, and develop individualized treatment plans to ease pain and promote healing. You will coach patients, maintain records, and participate in evidence-based interventions within a supportive health team.

You will supervise assistants and students, ensure regulatory compliance, and provide discharge summaries as part of comprehensive patient care.

Responsibilities

• Consult with patients to learn about their physical condition and symptoms and review their medical history and referrals from doctors or surgeons.
• Diagnose movement dysfunction by listening to patients and observing them as they move.
• Develop individualized treatment plan.
• Coach patient and teach therapeutic massage to ease pain and promote healing.
• Maintain patient records by tracking goals.
• Use standardized assessments and evidence-based practice to support clinical interventions.
• Assess patient progress and adjust treatments in the clinical record per regulations.
• Evaluate treatment outcomes and modify services based on outcomes and referrals.
• Provide consultation and counseling to patients and families and other service providers related to physical disorders.
• Communicate with supervisor and health team members about patient progress, barriers, and plans.
• Provide comprehensive discharge summaries per regulatory requirements.
• Document regulatory and clinical information in the electronic documentation system and maintain patient records.
• Participate in facility and company trainings, in-services and conferences.
• Supervise and support physical therapist assistants, rehabilitation aides and/or students.
• Adhere to company policies and code of conduct.
• Maintain active State Licensure.

• Utilize standardized assessments and evidence-based practice to support clinical interventions.
• Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
• Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
• Participate in facility and company required trainings, in-services and conferences.
• Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.
• Adherence to Company required policy and procedures and code of conduct.
• Active State Licensure
