Job summary

A growing Medicare-certified Home Health agency in O’ahu is seeking licensed Physical Therapists and Assistants for per diem roles. This position allows for flexible scheduling, focusing on providing in-home therapy to help seniors maintain their independence. Candidates must hold a current Hawaii license and have at least one year of clinical experience, ideally in home health. Strong documentation and communication skills are essential. Reliable transportation is required for this role.

Qualifications

• Current Hawaii license as a Physical Therapist or Physical Therapist Assistant.
• Minimum of 1 year clinical experience, preferably in home health.
• Must have reliable transportation.

Responsibilities

• Provide in-home physical therapy visits.
• Complete evaluations, reassessments, and fall risk assessments.
• Document accurately and communicate progress with the care team.

Pay:

• PT: $80 per visit
• PTA: $55-$60 per visit

Location: O’ahu, HI (reliable transportation required)
