Job summary

Alternate Solutions Health Network, LLC is actively seeking a Physical Therapist to deliver skilled home health services in the Cincinnati area. You will educate patients and families, ensure safety, and maintain compliant documentation while following physician orders.

The role requires at least one year of PT experience in acute care, with preferred home health exposure and a valid driver's license for in-home visits.

Qualifications

• Minimum one-year experience as a PT in an acute care setting.
• Home care experience preferred.
• Must work 1 dedicated weekend day per week.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands.
• Compassionate communicator with a positive attitude.
• Patience is a virtue when working with patients, families, physicians, and coworkers.

Responsibilities

• Provide skilled physical therapy as ordered by the attending physician.
• Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff.
• Perform OASIS assessments to develop an individualized plan of care and makes adjustments as needs change.
• Treats patient to relieve pain, develop or restore function, and maintain maximum performance using physical means, such as, exercise, massage, heat, water, etc.
• Helps decrease re-hospitalizations by front loading visits for high-risk patients and teaching the patient on preventative measures and making good decisions.
• Counsel patient and family on the disease/injury and how to manage in all phases of physical therapy.
• Instruct patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
• Documents all aspects of subsequent, discharge, eval/recert/resumption of care visits within 24 hours of visits.
• Oversee and supervise therapy services of patient provided by PTAs and when appropriate, home health aides.
• Communicate with the PTAs and home health aides on supervisory visit results.
• Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
• Operations: Attend in-service training and mandatory agency meetings.
• Other duties as assigned.

SUMMARY

The Physical Therapist (PT) provides comprehensive skilled services as ordered by the attending physician. The PT educates patients and their family members and ensures the safety of the patient.
