Alternate Solutions Health Network, LLC is seeking a Physical Therapist to provide skilled PT in patients' homes across Cleveland and surrounding areas. You will assess, treat, and educate patients and families to improve mobility, function, and quality of life under physician orders.
In this role you’ll complete evaluations, develop care plans, supervise PTAs as needed, and coordinate with care teams. A valid PT license in the state and driving is required; home health experience is preferred.
