Job summary

Alternate Solutions Health Network, LLC is seeking a Physical Therapist to provide skilled PT in patients' homes across Cleveland and surrounding areas. You will assess, treat, and educate patients and families to improve mobility, function, and quality of life under physician orders.

In this role you’ll complete evaluations, develop care plans, supervise PTAs as needed, and coordinate with care teams. A valid PT license in the state and driving is required; home health experience is preferred.

Qualifications

• Active Physical Therapist license (state of employment).
• 1+ year acute care PT experience required.
• Home health experience preferred.
• Strong communication, documentation, and clinical judgment.
• Valid driver’s license + auto insurance.

Responsibilities

• Complete evaluations and OASIS assessments.
• Develop and adjust individualized care plans.
• Provide skilled treatment and patient education.
• Monitor progress and communicate with care teams.
• Supervise PTAs and coordinate care as needed.
• Complete timely, accurate documentation per Medicare/agency guidelines.
• Support continuity of care and reduce hospital readmissions.

Role Overview
