Job summary

The Christ Hospital Home Health Care is seeking a Physical Therapist to deliver skilled therapy in patients' homes under physician orders and to educate patients and families while ensuring safe, compliant care and accurate documentation.

The PT will perform OASIS-based care planning, supervise PTAs and aides as needed, and communicate with the care team to adjust plans as patient needs change.

Qualifications

• Minimum one-year experience as a PT in an acute care setting.
• Must work one dedicated weekend day per week.
• Valid driver's license and auto insurance in your name.
• Capable of all physical demands.
• Compassionate communicator with a positive attitude.
• Patience when working with patients, families, physicians, and coworkers.
• Attention to detail and ability to follow directions.

Responsibilities

• Treat patients to relieve pain, develop or restore function, and maintain maximum performance using physical means.
• Supervise therapy services of PTAs and home health aides; communicate supervision results.
• Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.

Work Schedule: Tuesday Wed and SAT 815-5, on call every 8th -9th weekend

Summary

The Physical Therapist (PT) provides comprehensive skilled services as ordered by the attending physician. The PT educates patients and their family members and ensures the safety of the patient. The PT is accountable for completing accurate documentation and remains compliant with all legal rules and regulations.

Key Responsibilities

• Provide skilled physical therapy as ordered by the attending physician. Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff. Perform OASIS assessments to develop an individualized plan of care and makes adjustments as needs change.
• Treats patient to relieve pain, develop or restore function, and maintain maximum performance using physical means, such as, exercise, massage, heat, water, etc.
• Helps decrease re-hospitalizations by front loading visits for high-risk patients and teaching the patient on preventative measures and making good decisions.
• Counsel patient and family on the disease/injury and how to manage in all phases of physical therapy. Instruct patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
• Documents all aspects of subsequent, discharge, eval/recert/resumption of care visits within 24 hours of visits.
• Oversee and supervise therapy services of patient provided by PTAs and when appropriate, home health aides.
• Communicate with the PTAs and home health aides on supervisory visit results.
• Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
• Operations: Attend in-service training and mandatory agency meetings.
• Other duties as assigned.

Qualifications

• Minimum one-year experience as a PT in an acute care setting. Home care experience preferred.

EDUCATION AND CREDENTIALS

• Physical Therapist with a current license in the state of employment.
