Job summary

UnityPoint Health is seeking a Physical Therapist to join UnityPoint Clinic in Waterloo, IA. The role involves evaluating patients and creating individualized plans of care across outpatient settings.

You will provide interventions in line with standards, supervise PTAs and aides, and ensure timely EMR documentation. Collaboration in multidisciplinary teams is expected.

Qualifications

• Must hold current IA (Iowa) Physical Therapy licensure (or eligible).
• Current CPR certification.
• Education: degree from accredited Physical Therapy program (Certificate/Bachelor/Master/Doctorate).

Responsibilities

• Perform PT evaluation and develop plan of care based on patient impairments.
• Provide PT interventions per standards of care and policies.
• Supervise Physical Therapist Assistants and Rehabilitation Aides.
• Supervise and complete documentation for PT and PTA students as assigned.
• Document in EMR timely to demonstrate need for skilled interventions.
• Participate in patient care conferences and multidisciplinary staffing.

Responsibilities

• Perform Physical Therapy evaluation and develop the plan of care appropriate for each patient based on physical, mental, emotional, and developmental impairments and functional limitations. A variety of diagnoses and settings are served, including the three outpatient locations.
• Provide Physical Therapy interventions in accordance with established standards of care, policies, procedures, and state/federal law.
• Supervise Physical Therapist Assistants and Rehabilitation Aides.
• Supervise and complete appropriate documentation for Physical Therapy and Physical Therapy Assistant students as assigned by CCCE.
• Complete documentation in the electronic medical record in a timely manner in accordance with department and professional expectations, demonstrating the need for skilled interventions.
• Participate in patient care conferences and staffing as appropriate for multidisciplinary patient care.

Qualifications

Education:

• Certificate, Bachelor’s degree, Master’s degree, or Doctorate degree from an accredited Physical Therapy program.

License(s)/Certification(s):

• Current IA and IL Physical Therapy licensure (or eligible).
• Current CPR certification.
