Job summary

Jackson Therapy Partners, LLC seeks a traveling Physical Therapist to support Skilled Nursing Facility evaluations and treatments in Westfield, NJ. You will help patients recover mobility and function while adapting to new locations.

Requirements include a PT degree, NJ license, BLS, and experience in SNFs. The 13-week assignment runs 36–40 hours per week, with shifts 08:30–16:30, across days, traveling between sites.

Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active New Jersey Physical Therapist license required to start the assignment.
• BLS certification from American Heart Association or American Red Cross may be required.
• Experience working in a skilled nursing facility or long-term care setting is required.
• Demonstrated ability evaluating and treating adults with post-acute and chronic conditions, including mobility and functional restoration.
• Experience with documentation systems commonly used in SNFs and Medicare/Medicaid regulatory requirements.
• Ability to work independently and coordinate care with interdisciplinary teams.
• Preferred: prior travel PT experience and proficiency with restorative nursing programs.

Assignment Details

• Scheduled as 5x8 with a 30-minute meal break.
