Job summary

Lac Vieux Desert Health Center seeks a licensed Physical Therapist to provide patient-focused care under physician orders and a therapy plan of care. The role involves evaluating function, developing and updating plans, and delivering timely PT treatment in a collaborative setting.

Responsibilities include documenting progress, communicating with physicians, and educating patients and families in home programs. A strong team orientation and adherence to policies are required.

Qualifications

• Graduate from a physical therapy curriculum approved by the APTA or equivalent.

Responsibilities

• Assist the physician or mid-level provider in evaluating level of function and plan of care.
• Prepare clinical and progress notes.
• Provide physical therapy treatment to patients.
• Maintain teamwork and communicate progress to patients and staff.
• Write reports to physicians regarding patient progress.
• Instruct patients and families in home programs and activities of daily living.
• Adhere to policies and procedures and participate in care planning.
• Complete history and evaluation visit and obtain physician approval for plan of care.
