Job summary

Lac Vieux Desert Health Center is seeking a Physical Therapist to administer therapy under physician orders and the plan of care. The role supports evaluation, treatment, and documentation, working under supervision of the Director of Therapy Services.

Responsibilities include preparing notes, communicating progress, and collaborating with other healthcare staff. A Michigan PT license and BLS certification are required; two years of experience are preferred.

Qualifications

• Graduate from a physical therapy curriculum approved by the American Physical Therapy Association or equivalent.
• Must possess a current, full active and unrestricted license issued by the State of Michigan.
• Current Basic Life Support (BLS) training and certification.
• Two years' experience preferred.
• Ability to exercise initiative and independent judgement.

Responsibilities

• Assists the physician or mid-level provider in evaluating function and developing the plan of care.
• Prepares clinical and progress notes.
• Provides PT treatment to patients per physician orders.
• Maintains teamwork and good relations with staff.
• Communicates patient progress in reports and evaluations.
• Performs skilled PT procedures as ordered and revises plans as needed.
• Documents patient progress and plans for physician approval.
