Job summary

Haywood Post Acute is seeking a licensed Physical Therapist to evaluate residents, develop and implement individualized treatment plans, and supervise PT assistants. You will collaborate with physicians, nursing staff, and families to support mobility, safety, and discharge planning.

The role requires state licensure, strong documentation skills, and the ability to educate residents and caregivers on home exercise programs and safety techniques.

Qualifications

• Licensed Physical Therapist in the state with continuing education requirements.
• Degree in Physical Therapy; advanced degrees preferred.
• Proficient with PT documentation and electronic health records.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and function.
• Supervise PT assistants and coordinate with the care team.
• Communicate progress and discharge plans with physicians, families, and staff.
• Maintain compliant documentation and contribute to quality improvement.
