Job summary

The Chatham Hills Subacute Care Center is seeking an Experienced Physical Therapist to assess referred patients and provide direct patient care, developing and administering rehabilitation plans under physician direction.

You will document patient conditions, educate patients and families, and coordinate with the care team. Requires PT degree, state licensure, and experience in long-term/geriatric care; strong documentation and teamwork are essential.

Qualifications

• Must be licensed to practice Physical Therapy.
• Experience with long-term care and geriatric patients preferred.
• Excellent documentation and record-keeping skills.
• Collaborates with other disciplines to provide patient-centered care.

Responsibilities

• Assess referred patients to determine rehabilitation needs.
• Develop and administer treatment plans with goals aligned to patient capacity.
• Supervise and oversee Physical Therapy Assistants.
• Document patient progress and communicate with physicians and families.
