• Confer with care team and keep informed of treatment modifications and progress.
• Prepare Evaluation reports/summaries.
• Prepare Discharge reports/summaries.
• Provide Progress reports to referring physicians and agencies.

Education and Experience

• We WELCOME recent GRADS Education Experience
• Graduate of an approved school of Physical Therapy
• Must have Ohio State Licensure
• Skills Practice Physical Therapy consistent with the American Physical Therapy Association Code of Ethics and Standards of Practice
• Treat patients and function as a Physical Therapist as indicated
• Demonstrates excellent customer service
