Job summary

Talented Medical Solutions is seeking a Physical Therapist (PT) for a 117-bed skilled nursing facility in Baltimore, Maryland. This full-time role offers a 40-hour work week with Monday–Friday hours and occasional weekend rotation.

You will conduct evaluations, develop individualized treatment plans, and provide interventions to improve strength, mobility, balance, and independence for patients in long-term care and short-term rehab.

Qualifications

• Must hold Doctor of Physical Therapy (DPT) or equivalent degree.
• Current Maryland Physical Therapist license is required.
• Strong clinical judgment and communication skills are essential.
• New graduates are welcome to apply; compensation will reflect qualifications and experience.

Responsibilities

• Conduct physical therapy evaluations and functional assessments.
• Develop individualized treatment plans based on patient goals and clinical needs.
• Provide interventions to improve strength, mobility, balance, endurance, and independence.
• Treat patients in long-term care and short-term rehabilitation settings.
• Track patient progress and update plans of care as needed.
• Complete accurate and timely clinical documentation.
• Educate patients and caregivers on exercises, mobility, and safety.
• Collaborate with interdisciplinary team to support positive outcomes.
