Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in a Skilled Nursing Facility setting. You will evaluate and treat patients following an individualized plan of care, contribute to measurable functional goals, and collaborate with facility staff to deliver exceptional care.

The role involves supervising Physical Therapist Assistants, performing re-evaluations, and maintaining timely documentation to support patient progress and outcomes.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, addressing concerns and educating patients on injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Completing timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes
• Collaborating with facility staff to ensure high quality patient care
• Providing excellent care and customer service to patients and families
