Job summary

Maybrook Hills Rehab is seeking a full-time physical therapist for our skilled nursing facility. You will help residents regain independence and manage therapy plans across the continuum of care.

You will plan, develop, and deliver physical therapy services in compliance with state and federal regulations, monitor KPIs, coordinate evaluations and documentation, and collaborate closely with residents, families, and staff to ensure compassionate, quality care.

Qualifications

• Valid, unrestricted PT license in the state.
• Educational requirements for licensure.
• Valid driver’s license in the state of employment.
• Knowledgeable of skilled nursing home regulations and long-term care guidelines.

Responsibilities

• Plan, develop, and direct delivery of physical therapy services per laws and regulations.
• Coordinate PT indicators and reporting.
• Lead processes for screening, evaluation, treatment, discharge and documentation for residents.
• Provide resident assessment information including functional abilities, goals, ROM and balance.
• Maintain positive relationships with residents, staff and families.
• Ensure compassionate, quality care and maintain therapy spaces.
• Promote teamwork and effective communication.
