Elderwood of Lakeside at Brockport is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team in a skilled nursing setting. We offer full-time and part-time opportunities with flexible scheduling and a supportive, interdisciplinary environment focused on quality outcomes and meaningful resident progress.

Qualifications

• Graduation from an approved school of physical therapy required.
• State license to practice physical therapy required.
• Two years' experience in physical therapy preferred.
• Ability to direct others necessary.
• English language proficiency at intermediate level or higher required.

Responsabilités

• Provide skilled physical therapy evaluations and treatments in accordance with physician orders.
• Develop and update individualized care plans and therapy goals.
• Maintain timely and accurate documentation.
• Collaborate with interdisciplinary clinical teams regarding resident progress and needs.
• Participate in resident care planning meetings.
• Assist with education and training initiatives as needed.
• Support a safe, respectful, resident-centered care environment.
