Job summary

A skilled nursing facility in Florida is seeking a Full Time Physical Therapist responsible for providing and directing therapy services, coordinating treatment plans with patients and families, and conducting patient screenings. The ideal candidate must have a Bachelor's Degree in Physical Therapy, be licensed in the state of practice, and have one year of experience, preferably in a Skilled Nursing Facility. A flexible schedule including weekends and holidays is desirable.

Qualifications

• Bachelor's Degree in physical therapy from an accredited program.
• Completion of the National Certification Examination for Registered Physical Therapists.
• Physical therapy license in the state of practice.
• One year physical therapy experience, preferably in a Skilled Nursing Facility.
• Flexible schedule, including availability to work evenings, weekends and holidays.

Full Time Physical Therapist - Skilled Nursing Facility

Hours: 32-40 hours/week.

Provide and direct physical therapy services to patients, including assessment, treatment, program planning and implementation

• Work in accordance with physicians' orders and adhere to applicable principles and practices of physical therapy, Brookdale policies/procedures, and state regulations
• Conduct patients' screenings regularly to determine need for intervention and treatment
• Coordinate treatment plan with the patient, family and other team members
• Develop appropriate home or community programming to support the patient's continued recovery in his or her own environment

Qualifications

• Bachelor's Degree in physical therapy from an accredited program
• Completion of the National Certification Examination for Registered Physical Therapists
• Physical therapy license in the state of practice
• One year physical therapy experience - prefer Skilled Nursing Facility Experience
• Flexible schedule, including availability to work evenings, weekends and holidays
