Job summary

Trinity Health Of New England At Home is seeking a dedicated Physical Therapist to deliver in-home care, collaborating with physicians, nurses, social workers, and aides to provide patient-centered therapy. The role emphasizes autonomy, precise documentation, and adherence to laws and policies.

The position offers a $10,000 sign-on bonus paid in installments and a Monday–Friday schedule with rotating weekends, reflecting a commitment to high-quality, home-based rehabilitation.

Qualifications

• Degree including internship from an APTA-approved program.
• Current Physical Therapist license in Connecticut.
• 1 year minimum as PT or completion of 6–8 week preceptorship.
• Familiarity with home health care and regulatory requirements preferred.
• Maintain clinical skills and adhere to organizational policies.

Responsibilities

• Plan and prioritize visits based on client needs and physician orders.
• Assess clients at admission and key intervals and document accurately.
• Develop and update care plans with measurable goals with physician and client.
• Provide therapy using approved techniques, exercises, and modalities.
• Coordinate care and referrals with other disciplines as needed.
• Educate clients and caregivers on therapy programs and home exercises.
• Monitor progress and report changes to the physician.
• Ensure compliance with laws, regulations, and agency policies.
• Complete documentation at point of care in a timely manner.
