Job summary

CommuniCare Rehab, a member of CommuniCare Family of Companies, has a full-time Physical Therapist (PT) opening at Mercer Healthcare Center in Bluefield, WV. This position is an in-house team member role, focused on improving residents’ quality of life through therapy.

Qualifications include a degree in Physical Therapy (Bachelor’s, Master’s, or Doctorate) and a valid or pending state license. Responsibilities include chart reviews, treatment planning, supervising PTAs, and participating in

Qualifications

• A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License.

Responsibilities

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other rehab team members, and conduct staff/family training and education when appropriate.
