Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this assessment, the therapist develops and implements treatment plans, performs interventions to improve patient well-being, and monitors progress.

• Evaluate the patient's functional status and physical therapy needs, assisting physicians in developing the therapy plan of care.
• Observe and report patient responses to treatment and any changes in condition to the supervising nurse and/or physician.
• Treat patients to relieve pain, restore or develop function, and maintain maximum functioning according to accepted standards of practice.
• Educate and inform the healthcare team, patients, and families about the plan of care, functional limitations, and progress toward goals.

Education and Experience

Licensure Requirements

• Current Physical Therapy licensure in the state of practice.
• Current CPR certification required.
• Valid driver's license, vehicle insurance, and access to a dependable vehicle.
