Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this assessment, the therapist develops a treatment plan, performs interventions to improve patient well-being, and evaluates progress.

• Evaluate the patient's functional status and physical therapy needs, assisting in developing the therapy plan of care in collaboration with the physician.
• Monitor and report the patient's response to treatment and any changes in condition to the supervising nurse and/or physician.
• Treat patients to relieve pain, restore or develop function, and maintain maximum functioning according to accepted standards.
• Educate and inform the healthcare team, patients, and families/caregivers about the plan of care, functional limitations, and progress towards goals.

Education and Experience

Licensure Requirements:

• Current Physical Therapy license in the state of practice.
• Current CPR certification.
• A valid driver's license, vehicle insurance, and access to a dependable vehicle.
