Job summary

H2 Health Prime Living in the Mandarin area of Jacksonville is seeking a Physical Therapist for 1:1 patient care in a luxury senior living setting. This PRN position offers a flexible schedule, clinician-led and patient-focused environment, and opportunity to grow within a supportive team.

New graduates are welcome; active or eligible license required. Responsibilities include evaluating, treating, documenting progress in EMR, and collaborating with a multidisciplinary team to deliver

• Must have a degree from an accredited Physical Therapy program.
• Active or eligible state license to practice as a Physical Therapist (PT).
• New graduates welcome; outpatient experience valued.

Responsibilities

• Evaluate and treat patients with orthopedic, post-operative, neurological, and sports-related conditions in an outpatient setting.
• Design and implement personalized treatment plans to restore function and improve quality of life.
• Document patient progress accurately using EMR.
• Collaborate with a multidisciplinary team to deliver holistic, patient-centered care.
• Contribute to a positive, professional clinic environment reflecting the company culture.
