Job summary

H2 Health Prime Living is seeking a Physical Therapist to deliver 1:1 patient care in luxury senior living communities in Pensacola. You’ll work under a clinician-led, patient-focused model, with opportunities to grow through mentorship and ongoing education.

Qualifications

• Education: degree from an accredited Physical Therapy or Physical Therapist Assistant program.
• License: active or eligible state license to practice as a Physical Therapist (PT) or Physical Therapist Assistant.
• Experience: new grads are welcome; outpatient experience is a plus.

Responsibilities

• Evaluate and treat patients with orthopedic, post-operative, neurological, and sports-related conditions in an outpatient setting.
• Design and implement personalized treatment plans aimed at restoring function and enhancing quality of life.
• Document patient progress accurately using EMR.
• Collaborate with a multidisciplinary team to deliver holistic, patient-centered care.
• Contribute to a positive, clean, and professional clinic environment that reflects H2 Health’s culture of excellence.
