Job summary

Maryland Gardens Post Acute is seeking a Staff Physical Therapist to evaluate and treat residents, coordinate with families and care teams, and maintain comprehensive documentation. The role includes supervising PT Assistants, training staff, and contributing to care conferences and quality initiatives.

The ideal candidate holds a Bachelor’s degree in Physical Therapy with preference for those with Masters or Doctorate.

Qualifications

• Bachelor’s degree in Physical Therapy required; Master’s/Doctorate preferred.
• Proficient in computer data entry for Net Health and Point Click Care.

Responsibilities

• Evaluate residents to determine physical therapy needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT Assistants in resident care and related activities.
• Communicate progress and discharge plans with physicians, nursing staff, residents and families.
• Complete timely, accurate documentation per facility and regulatory requirements.
• Educate residents and families on goals, home exercise programs, and safety techniques.
