Job summary

Maryland-Gardens-Post-Acute is seeking a dedicated Staff Physical Therapist to assess residents, design and update individualized treatment plans, and lead mobility-focused rehabilitation in a long-term care setting.

You will collaborate with physicians, nursing staff and families, deliver in-services, and supervise PT assistants while maintaining compliant documentation and patient safety standards.

Qualifications

• Bachelor's degree in Physical Therapy is required.
• Master's or Doctorate in PT preferred.
• Licensed Physical Therapist in the state; license in good standing.
• Continuing education requirements must be maintained.

Responsibilities

• Evaluate residents to determine PT needs and rehab potential.
• Develop and update individualized treatment plans to improve mobility.
• Implement plans to enhance strength, balance, endurance, and independence.
• Supervise PT assistants and students; train staff as needed.
• Communicate progress, goals, and discharge plans with physicians, families and team.
• Complete timely documentation per facility and payer requirements.
• Educate residents, families and caregivers on goals, home programs and safety.
• Participate in RCF meetings, QA initiatives, and rehab program development.
