Job summary

One Senior Care is seeking a Physical Therapist to provide accurate, evidence-based therapy to Participants across centers, homes, and facilities.

You'll perform comprehensive evaluations, design individualized plans, and deliver therapeutic exercise, gait training, and modalities to maintain mobility and independence. Collaboration with care teams and training of caregivers are key parts of the role.

Qualifications

• Must hold a current PT license in the state you are practicing in.
• Experience with geriatric population preferred; training provided.
• Strong clinical assessment and documentation skills.

Responsibilities

• Conduct comprehensive PT assessments and ongoing evaluations.
• Review medical history; establish individualized plans of care and goals.
• Provide therapeutic exercise, gait training, and modalities.
• Document evaluations and progress in EMR.
• Supervise PTAs as required; train caregivers and staff.
• Deliver therapy across center, homes, SNFs, and hospitals.
