Salem Transitional Care is seeking a Physical Therapist who is passionate about helping residents build strength, regain mobility, and reach goals that improve daily life.

You’ll evaluate residents, develop personalized plans, collaborate with an interdisciplinary team, educate families, and ensure HIPAA-compliant documentation while enjoying a supportive workplace with relocation assistance and a sign-on bonus.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Set realistic therapy goals based on capacity and tolerance.
• Deliver interventions to improve mobility, strength, and function.
• Monitor progress and adjust plans to meet goals.
• Collaborate with nursing, therapy, and other disciplines.
• Educate residents and families on therapy methods.
• Document progress per state and federal requirements.
• Provide supervision to Physical Therapist Assistants.
