Job summary

Hopedale Medical Complex is seeking a compassionate and motivated Physical Therapist to join our Rehabilitation Services team. Evaluate, develop, and implement individualized treatment plans to help patients restore function, improve mobility, reduce pain, and maximize independence.

As part of a multidisciplinary rehabilitation team, you will collaborate with physicians, nursing staff, and other therapists to deliver patient-centered care across inpatient, outpatient, or long-term care settings,

Qualifications

• Graduate of an accredited Physical Therapy program (DPT preferred).
• Current Illinois Physical Therapist license or eligibility for licensure.
• Current CPR/BLS certification or ability to obtain upon hire.
• One to two years of clinical experience preferred; new graduates are encouraged to apply.
• Strong clinical assessment, communication, and critical thinking skills.
• Ability to work effectively within a collaborative healthcare team.
• Commitment to delivering compassionate, patient-centered care.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide skilled physical therapy treatments using evidence-based techniques.
• Monitor patient progress and modify treatment plans as appropriate.
• Educate patients and caregivers on home exercise programs, mobility, safety, and use of assistive devices.
• Collaborate with physicians and interdisciplinary healthcare teams to optimize patient outcomes.
• Complete accurate and timely clinical documentation.
• Participate in discharge planning and continuity of care.
• Mentor Physical Therapist Assistants, students, and rehabilitation support staff as appropriate.
• Participate in quality improvement initiatives and continuing education.
• Maintain compliance with HIPAA, accreditation standards, and HMC policies.
